What is it?
The Fillable Form Generator turns your questions into a PDF form that people can type into on a phone or computer. It can also make a filled and locked copy, or HTML code for a website form.
Who is it for?
Schools, small businesses, NGOs, clubs and offices that need registration, feedback, order or admission forms as fillable PDFs, without buying PDF software.
Build the form
- Start from a Template or a blank form. Write the Form title and Instructions.
- Tap + Add question. Choose its Type: Short answer, Long answer, Number, Date, Email, Phone, Choose one, Tick boxes, Drop-down, Yes / No, Signature line or Section heading. For choices, type the Choices, separated by commas.
- Download fillable PDF to share it, or fill it here and Download filled PDF, optionally locked, cannot be edited. Copy HTML code gives a web version.
The PDF won’t fill on someone’s phone
- Some viewers, such as image previews in WhatsApp or Gmail, only show the PDF. Open it in a PDF app (Adobe Acrobat Reader, Google Drive’s PDF viewer, or a browser) to type in it.
- After filling, they must Save or Download the PDF again, or their answers are lost.
Good to know
- The signature line is a space to sign on paper or with a PDF app’s signature feature.
- Use the locked filled PDF when you send a final copy that shouldn’t be changed.
- To collect answers from many people automatically, an online form service is better; this tool is for PDFs you send and receive.
