What is it?
Affidavit and Declaration Formats gives you ready text for common affidavits that you can edit and print. It covers self-declaration, name change, address proof, date of birth, gap year and lost documents, plus UK and US styles.
Who is it for?
Students needing gap-year affidavits, people changing names after marriage, applicants for passports and documents, and anyone asked for “an affidavit on stamp paper”.
How to use it
- Choose the kind of affidavit or declaration.
- Fill in your details; the text updates.
- Copy or print it, then get it typed on stamp paper (if required) and attested by a notary or oath commissioner.
Affidavit basics (India)
An affidavit is a sworn statement: it usually goes on non-judicial stamp paper of the value your state requires, is signed in front of a notary or oath commissioner, and must be true; false affidavits are an offence. A self-declaration is simpler and often needs only your signature.
These are drafts to save time, not legal advice. Laws, stamp duty and notary rules differ by state and country; have important documents checked by a lawyer or notary.
Errors and how to fix them
- Office rejects the format: some offices want their own format; ask for it and use this only as a guide.
- Header and date on the printout: untick “Headers and footers” in the print window.
